The therapist comes when your sim drops into Aspiration Failure. Only that sim can see the shrink, so everyone else thinks he's crazy when he starts acting like an animal.

BTW, Blind Dates will also drop out of the sky if you ask the gypsy for one.

Yeah I like watching the social bunny and the therapist from another sims point of view. When you're controling another sim you can't see them so it's so funny to watch that sim do all these crazy things and interact with an invisible sim.
