The cosmetic surgery thing is a career reward item. Specifically it is found when someone in the Show Business career track gets to level 6 (Supporting Player). This is one of the careers which is only open to Sims who have graduated from university. On the university lots, it can be found on Secret Society Lot 1 (or if you use the Maxis neighborhoods, Academie Le Tour uses that lot for its Secret Society.
