To create a sim, open the "CAS" (create-a-sim), in the neighborhood screen. This is found under the "families" button. To start, click the button that looks like a “blank” head, and select the option “Create a Sim”. This will load a completely random sim. This is what you have to work with. In this panel, you can change your sim’s age, body/fitness type, gender and skin tone.
You can also access a pre-made sim from the “sim bin” (Maxis sims, sims made in BodyShop, and downloaded sims, the latter two will be marked with an “*”), and “randomize” your sim. In the next panel, you can select a pre-made head. The actual heads available vary depending on your gender/skin tone/age selections made in the first panel. You are now ready to fine-tune the facial structure and coloring of your sim. On the left hand side, there are several areas to focus on (hair, jaw, chin etc.). Each one has many options pertaining to that particular aspect of the selected feature. In addition to the “sliders” under the facial features, you can click on pre-made faces/facial parts. Right-clicking on these will make your sim’s face just a little bit like the picture. The next panel deals with makeup, facial hair and glasses. Here you can adjust make-up (blush, eyeliner, lipcolor, etc.),costume makeup (elders also can add more prominent wrinkles here), stubble (“five o’clock shadows”), and facial hair. Note: Females cannot have facial hair or stubble,( for obvious reasons, although in real-life, there are the occasional “bearded ladies” lol), children and toddler can’t have facial hair either, and toddlers can’t wear regular makeup, though they can wear costume makeup. The next panel controls clothing. The tabs open up your options for formalwear, everyday clothes, swimwear (toddlers can’t wear bathing suits), under wear and activewear (kids and toddlers can’t have any activewear.). Under each tab, the options for that clothing type are shown. The everyday wear has two options: Full body suits, and Mix, under which you separately choose a top and bottom to your sim’s outfit. In the last panel, you will find controls for your sim’s personality, aspiration, and zodiac sign. You can select a zodiac sign for a random personality, or create one by adding personality points under each characteristic (i.e. Serious/playful, lazy/active etc.). Choose your aspiration.
When you are done with everything, select the “√”.. If you make multiple sims (click , you can change their relationships (child/parent, spouse, sibling, roomie) to each other under the “family relationship tool” (this looks like a graph thingy, it’s below the family symbol on the bottom left side of the screen.) Drag the sim’s icon over another’s the establish a relationship. You can always go back and edit (or delete) a sim by clicking their icon and selecting “Edit This Sim”. When you are done, click the check at the left hand bottom side of the screen, and your family is done. Please note that you cannot change many parts of your sims after you click this button, so make sure you are satisfied with your family and sims before clicking this. After waiting for this family to load, you will be back in the neighborhood screen, and your sim/family will be in the family bin. Just move them into a house or lot to play.

Also, BodyShop is a program that comes with TS2, and can be used to make sims, and also upload them to thesims2.ea.com. It is very useful, but can be hard to find. In my computer, it is in ProgramFiles>EAGames>TheSims2>CSbin. There are many tutorials online which can help you with getting to use this program. Here are some links you may find useful:

    Good luck, and have fun
